INDEPENDENT MEDICAL EVALUATION (IME)
An independent medical evaluation (IME) is an assessment performed by a physician who does not treat the patient. Independent medical evaluators are retained by the requesting party. Therefore, their relationship with the patient is different from the relationship in the traditional physician–patient model. Addiction and other mental health disorders, when they occur in healthcare professionals, take on specific characteristics that require specialized providers to perform an assessment to determine the nature of the problem.

An IME for addiction or mental health in healthcare professionals often involves a comprehensive assessment that includes an in-depth review of the individual’s work history, access to medications, and any patterns of substance use or mental health concerns. The evaluator may also assess the individual’s ability to safely return to work, their risk for relapse, and the potential impact of their condition on their ability to perform their professional duties without endangering patients or themselves.
These evaluations often include a review of the professional’s medical and psychiatric history, interviews with colleagues or supervisors (when necessary), and various diagnostic tools, such as drug or alcohol screenings and psychological assessments. The goal of the IME is to determine the severity and nature of the problem, whether it’s substance use, burnout, or another psychiatric issue, and to provide a clear and objective opinion on the healthcare professional’s fitness for duty.
